365Karate Participation Agreement, Assumption of Risk & Adult Release
1. Who is agreeing I am at least 18 years old. I am agreeing for myself if I participate, and I am the parent or legal guardian authorized to consent for each minor Student selected in this signing session. I cannot sign for another adult; each participating adult must provide their own agreement. Buying a gift does not consent to anyone else's participation.
2. Online instruction and supervision 365Karate provides online martial-arts instruction and practice activities. An instructor is not physically present to inspect our space, supervise us, or correct every movement. I will actively supervise participating minors and choose activities suitable for each Student's age, ability, and experience. Online instruction does not replace individualized medical advice or qualified in-person supervision where needed.
3. Risks and voluntary participation I understand martial-arts and fitness practice involve risks, including falls, collisions, overexertion, strains, sprains, fractures, head or neck injury, and other serious injury or death. Injuries may result from movements, surroundings, equipment, other participants, or following or misunderstanding instruction. I understand reasonable precautions cannot eliminate all risks. Participation is voluntary, and any Student may stop or skip an activity.
4. Safe practice I will provide a clear practice area, suitable footing, and appropriate equipment. I will follow safety instructions, avoid contact or partner techniques without suitable supervision, and not attempt weapons or advanced activities beyond the Student's training. I will seek qualified medical advice when health conditions or other concerns make participation uncertain. We will stop if an activity causes pain, dizziness, breathing difficulty, or other concerning symptoms. In an emergency, I will contact local emergency services; 365Karate does not monitor our practice or provide emergency care.
5. Adult assumption of risk and release For my own participation, I voluntarily assume the risks described above. To the extent permitted by applicable law, I release 365Karate, and its instructors and personnel acting in that capacity, from claims for personal injury or property damage arising from my participation, including claims based on their ordinary negligence. This does not release gross negligence, reckless or intentional misconduct, or liability or rights that cannot lawfully be waived. This adult release does not purport to waive a minor's independent legal rights.
6. Minor participation I authorize the selected minor Students to participate under my supervision and acknowledge the risks described above. I will explain the safety rules in a way they understand and stop activities I consider unsuitable. This consent is not a promise that every activity is appropriate for every child.
7. Program recognition and separate choices Ranks and certificates recognize progress within the 365Karate online program; acceptance by another organization is not guaranteed. This agreement does not authorize a charge, recurring subscription, marketing messages, or public use of a Student's name, image, or recordings. Purchase terms and privacy choices are presented separately.
8. Acknowledgment and signature I have read this agreement, understand that the adult release affects legal rights, and have had the opportunity to ask questions before signing. I voluntarily agree and intend my electronic signature to sign this exact version. I can save or print a copy. Material changes require a new explicit acceptance.
